Long tiles are held: press and keep holding until the tail clears the line.
PERFECT lands dead on the line, GREAT is close, GOOD still counts.
Every hit builds your combo. Reach 24 and the stage hits FEVER: double points until you miss.
Missing drains the bar at the top. Empty it and the run ends.
On a keyboard the lanes are D F J K (or the arrow keys).

Trouble hitting PERFECT? Open Settings and drag the timing offset until it feels right. Wireless headphones almost always need a positive offset.
